A qualifier is a condition (e.g. "weapon-context") that a phrase-watch or runbook can be gated on.
Runbooks
A runbook is a protocol map — a tree of question nodes whose answers decide the next question. A runbook with no qualifiers applies to every call; a gated runbook applies once its qualifiers match. (Tree authoring lands with Changes 2–6; for now a runbook is authored as its name, description and qualifier wiring.)
Phrases
A phrase is the thing detected (e.g. "mentions a weapon"). A phrase-watch puts a phrase in play globally, on every call or gated by qualifiers, independent of any runbook.

A phrase-watch with no qualifiers is in play on every call — this is where weapons and self-harm live. With qualifiers it is gated, only in play once all its qualifiers are active.
